Health promotion, disease and injury prevention and chronic disease management to clients
Works to promote overall health and well-being and improve access to health care and psychiatric services in the community * group and one-on-one services
Diabetes Screening Program - Early detection and screening for diabetes with follow-up with appropriate health care professionals
Diabetes Support Clinics - Monthly clinics for those who have pre-diabetes or diabetes to assist in the implementation of healthier lifestyle changes
Foot Care Clinics - Assessment, treatment, and education for those at risk of developing foot problems
Medical Clinic - Health care to those without access to a doctor in the community * coordinates care with psychiatrist and other team members * promotes participation in health care through health promotion * doctor available two half days per week to see clients * staff able to consult doctor by phone or email five days per week
Nutrition Clinics - Services to help prevent and treat illnesses by promotion of healthy eating habits and suggesting diet modifications
Occupational Therapy - Helps people to succeed at activities they want or need to do * person, activity and environment are assessed * one-on-one and group interventions may be directed at making adjustments
