Addiction Transitional Case Management Service - Assessment and Referral
- Program facilitates discharge process from London Health Sciences Centre and St Thomas Elgin General Hospital * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care

- Delivers smoking cessation treatment and counselling support to registered clients who wish to quit smoking
