Description des services: Interdisciplinary team that provides services and supports for older adults, their families and caregivers, who are coping with or at risk for developing responsive behaviours associated with dementia, mental illness, addictions and other neurological conditions

Services include:

  • assessment and screening
  • plan, implement, and evaluate care strategies
  • document and promote effective communication between the person with responsive behaviours, their families, mobile team members, and all other care providers
  • short-term follow up
  • education and support to staff in long term care, hospital, and the community, as well as families and other care partners
