Community Support Services (CSS) Central Intake connects individuals in London and Middlesex to multiple community support services through one intake process.
Meals and Nutrition: Community dining and hot or frozen Meals on Wheels.
Education and Support: Adult day programs, caregiver support, education, and service navigation.
Safety and Reassurance: Check-in visits and calls, emergency response systems, and hospital discharge support.
Health and Wellness: Social activities, fitness, bathing, medication support, and falls prevention.
In-Home Support: Personal care, housekeeping, meal preparation, respite, and caregiver relief.
Transportation: Rides to medical appointments and, when available, social activities and shopping.
Intensive Support: Assisted living, respite care, supportive housing, and seniors’ support programs.
Specialized Services: Support for people with dementia, acquired brain injuries, disabilities, and their caregivers.
