Community health care agency * Personal Support
Manager available to complete free in-home assessment to establish care plan
that meets patients’ needs to remain independent in own
home.
In-Home Personal Support Workers provide:
- bathing/assistance with personal care
- dressing assistance
- mobility assistance
- meal preparation assistance
Nursing services are provided at designated clinic locations * please call our
office for more information * nursing care provided to our clinics includes:
- wound care
- IV Therapy
- catheter
- drain/staple/suture care
- health teaching regarding diagnosis/disease
self-management/medication management
Effective April 22/2025 our Wingham clinic will relocate to Royal Oaks Wellness
Centre located at 131 John Street East, Room 3, Wingham Ontario. Parking
lot is located off the Frances Street entrance).
Our Stratford clinic is located at 61 Lorne Ave East unit 4 Stratford Ontario.
Nursing care provided in-home for patients who meet exclusion criteria for
clinic.
Therapy services provided in Huron & Perth include:

- dietary
- speech SLP
